For more than 40 years, Hill Physicians Medical Group has made it possible for independent private practices to be successful in an ever-changing, complicated and highly regulated environment, side by side in communities also served by large institutions.
Of the 1,800 practices that make up Hill Physicians, from San Francisco to Nevada County, the majority are small practices with fewer than 10 doctors, and many of these give all new meaning to the phrase “Family Medicine.” They are staffed by doctors, clinicians and front office managers who are, in fact, family members. We are proud to share some of their stories.

Although independent practices are the backbone of Hill Physicians, we could not enjoy the success we do today without the great working relationships we have with our largest strategic partners, according to David Joyner, CEO.
These systems not only help us expand access to specialty care for our patients, they also serve as an anchor in many of our regions and often provide hospitals and urgent care facilities for our patients, further expanding options.
Here’s a quick look at some of the success our partnerships have brought to the network:
UCSF: Since 2019, the Hill Physicians’ risk adjustment team has worked closely with UCSF to significantly improve the accuracy of chronic condition coding among Medicare Advantage patients.
As one of our largest practices, UCSF’s collaboration has been vital. Accurate coding ensures our patients’ medical records reflect their complete health history, leading to better care coordination, timely follow-up and fewer delays in tests, treatments or referrals.
Avi Tutman, Manager of Risk Capture at UCSF Health, shared, “UCSF Health greatly appreciates our partnership with Hill Physicians. Together, we’ve bridged data system gaps, launched AI and other technologies to reduce clinician burden, and organized high-profile community wellness events for Hill Physician members. These efforts have brought our risk capture rates to better serve patients with chronic conditions and needs.”
NorthBay Health: NorthBay Health’s team of roughly 170 primary and specialty care doctors joined the Hill Physician Network in 2021.
“Our partnership with Hill Physicians Medical Group has been instrumental in strengthening our ability to deliver comprehensive, high-quality care across the communities we serve,” said Seth Kaufman, M.D., Executive Vice President, Chief Medical Officer and Chief Quality Officer.
“As an independent health system, NorthBay Health values collaboration that expands access while preserving a patient-centered approach,” he added. “Hill Physicians’ network complements our services in a meaningful way, allowing us to better coordinate care and meet patients where they are.”
Dignity Health – Stockton: According to Alondra Cervantes, Dignity Health’s Director of Operations in Stockton, the partnership between Hill Physicians and Dignity Health – Stockton helps both organizations better serve the community through seamless, high-quality care.
“This collaboration exemplifies what’s possible when organizations align around a shared mission: delivering high-quality, patient-centered care through strong partnership and innovation,” she said.
Washington Health: Reynaldo Art Gomez, Director of Quality and Compliance, says a shared commitment to quality health care has been the basis for a solid relationship with Hill Physicians since 2007. In addition to a 415-bed acute care hospital, the Fremont-based health system boasts more than 20 primary care physicians and more than 40 specialists in the Hill Physician network, ensuring ample access to care for patients in and around Fremont.
“Our data-driven collaboration has turned shared values into measurable success,” he noted. “We have aligned our priorities and made a commitment to continuous improvement to provide high standards of outpatient care to our community.”

Dedication to Shared Success
In 2025, Hill Physicians Chief Medical Officer K. Gabrielle Gaspar, M.D., launched a listening tour, visiting physicians in person and virtually across our 14-county market in Northern California, asking the network of roughly 6,000 providers what issues matter most.
The good news, she said, is that Hill Physicians was already on track to meet the physicians’ stated needs and strengthen areas that could benefit from extra effort.
“It’s a win-win that our commitments and our providers’ needs are so clearly aligned,” said Dr. Gaspar. “Our Physician Compact highlights the many resources that Hill Physicians provides to doctors in its network, and illustrates that we are not only listening, but that we’re already on the same page,” said Dr. Gaspar.
Dr. Gaspar continues to invite doctors to sign their pledge, which is a commitment to delivering excellent, timely and evidence-based care, prioritizing network care as appropriate, communicating with colleagues and Hill Physicians staff collaboratively and respectfully, and being a positive ambassador for Hill Physicians.
“There are really no surprises in the Compact, it just reiterates the shared goals and responsibilities that make our network – and our physicians – successful,” she said.
