 Strategic Plan Aligns Core Values
with Growth and Development

Good companies roll with the punches. Great companies plan for them. That’s why Hill Physicians Medical Group created a strategic plan in 2024 that will guide us through an ever-changing healthcare landscape for the next three to five years.

“With so much change in health care, organizations like ours must always be on their toes, quick to pick up on emerging trends and translate them into workable solutions,” said CEO David Joyner. “That means listening to the needs of our physicians and their practices, staying on top of the latest technologies, working with health plans and other partners, and translating it all into programs and resources that ensure the patient receives the best care possible.” 

The plan that emerged is a multiyear strategy divided into four core goals: to solidify our physician network, grow our membership base, enhance our population health capabilities and strengthen our alignment with key partners.

Solidifying the Network: Independent physicians are the backbone of the Hill Physicians network; however, as longtime members of Hill Physicians near retirement, we are facing the prospect of a shrinking network. Thankfully, our successful recruitment efforts in the past three years demonstrate that we can confront this challenge successfully. Our strategy will be to devote more resources to physician recruitment. We will target strong and growing practices that will form the pillars for our network for years to come.

Growing our Membership Base: A diverse membership base across multiple lines of business is key to our success. Government lines of business (Medicare, Medi-Cal and dual-eligible programs) are especially ripe for growth and we will position ourselves to take advantage. We will also focus on growing our Clinically Integrated Network, Covered California and CalPERS membership, and partner with health plans on commercial HMO initiatives. We plan to continue our collaboration with Aledade in the Medicare Shared Savings Program (MSSP), delivering high-quality care to thousands of Medicare patients.

Enhancing Population Health Capabilities: A cornerstone to our success is our comprehensive approach to Population Health — keeping our members healthy and enabling access to the right care for their needs. Our team of nurses, pharmacists, behavioral health specialists and social workers work directly with members and physicians to ensure timely health screenings, cost-effective medications and manage the management of chronic conditions. We will continue to invest in technology and analytics to ensure our clinical services teams are as effective as possible in collaborating with our physicians and members.

Strengthen Alignment with Key Partners: Achieving our goals requires strong relationships with aligned partners — health plans, hospital systems and physician groups. We will prioritize long-term durable alignment and institutional relationships to bring the most attractive product offerings to market, collaborate on care initiatives for our patients and support the independent practice community.

Growth by the Numbers

If there was a theme to 2024, it would be “Growth.”

Hill Physicians Medical Group experienced growth in every significant segment of its business, from its physician network to its membership base of patients, and in its business lines of service throughout its growing footprint.

The charts offer perspective of that growth. Here are some highlights:

Providers

The number of providers in our network grew 7.9 percent, to 6,249 in 2024.

We saw the number of practicing Hill Physicians providers increase in five of six regions: The East Bay, Marin-Sonoma, Sacramento, San Francisco and Solano.

In 2024, Hill Physician’s provider network included 1,286 primary care physicians, 4,728 specialists and consultants; 161 Advanced Practice Providers and 74 physicians who are both primary care and specialty physicians.

Enrollment

Hill Physicians has experienced steady growth in membership, growing by 7.6% from 2023 to 2024. By the end of 2024, membership was just shy of 400,000 across all lines of business.

Revenue

In 2024, Hill recorded $831 million in revenue, compared to $761 million in 2024, an increase of 9.1 percent, and the highest revenues in Hill Physicians’ history, a 62 percent increase from the $512 million recorded in 2014.

Expansion into Marin and Sonoma

With a longtime presence in the greater Bay Area, Marin County was a natural expansion opportunity for Hill Physicians. Throughout 2024, Hill Physicians worked to build the physician network and establish contracts with key health plans in the county.

“We’re grateful for the partnership with Marin Health, which brought about 250 physicians into our network,” explained Mitra Javidi, Chief Contracting Officer. “Our partnership with Canopy and Western Health Advantage allowed us to enter the market with more than 5,000 patients by Jan. 1, 2025.”

In addition to the physicians associated with Marin Health, there were dozens of individual physicians and small practices in Marin and Sonoma that recognized the value of joining the Hill Network, said Mitra, and it continues to grow.

“We’re excited to serve the Marin community and expect to see continued growth as more and more health plans seek to contract with Hill Physicians in Marin, Sonoma, and perhaps beyond,” said Mitra.

In 2024, Hill Physicians Medical Group significantly increased our online presence with a 66 percent increase in visits to our website and a 12 percent increase in online followers. We brought in 318,000 site users, a 64 percent increase. We also added another 1,244 Facebook followers, bringing us to 11,192 by the end of the year. We believe the key to our success was using fresh, engaging creative content, with a light, fun approach, well-suited for engagement, putting emphasis on our diverse patient base.
